If you are a new patient, you can call us at (707) 254-1770. If you are an existing patient, you can schedule an appointment by calling or by sending a message online through our patient portal.
You can send a message to your provider through the patient portal. You may also call (707) 254-1770 and leave a message. Your call will be returned, usually by your provider’s nurse, within one business day.
To request a prescription refill, please call the pharmacy where you picked up your medicine. The phone number is on your prescription bottle. If no refill is available, please call (707) 254-1770. Once we receive your request, your prescription will be ready within 3 business days.
Your care team will contact you when you lab results are in.
If you’re registered with the portal, you can review your lab results as soon
as they’re ready.

Yes. If you are uninsured, we charge for services on a sliding fee scale, based on your family income. We can also help you apply for insurance or reapply if you have lost coverage. Please call us at (707) 254-1777.

To begin using the OLE Health patient portal, you will need to request access. You can do that during an appointment or by calling 707-254-1770. You will receive an automated email with login instructions. Make sure to check your spam folder if you don’t see it.
You can download your medical records, progress notes, and other health information via the online patient portal.
You can also request copies of health information from the Health Information Department by completing the Release/Request of Information form:

We report all vaccines to the State Department of Health. You can request your digital vaccine record online from the state of California at anytime by visiting http://myvaccinerecord.cdph.ca.gov/ and filling out the requested information.
